National Council on Aging Launches Toolkit to Promote and Expand Evidence-Based Programs in Rural Communities

On Nov 12, the National Council on Aging launched a new toolkit for community-based organizations (CBOs) designed to provide a roadmap for promoting and expanding evidence-based programs for health promotion and chronic disease self-management in rural communities.

Via the NCOA,

Evidence-based health education, including chronic disease self-management programs, is vital in complementing clinical care and building everyday skills to improve quality of life. And this education can be particularly valuable for rural communities where challenges exist when it comes to accessing health care and community-based services.

This toolkit is intended to work in conjunction with NCOA’s EBP search tool, which allows providers to identify programs that could benefit the populations and communities they serve, and provides recommendations regarding best practices for “building and advancing community partnerships, enhancing participant engagement, adapting and effectively delivering evidence-based programs, [and] overcoming key barriers in rural communities, such as transportation and connectivity for virtual access to care.”
